When auscultating the heart, which area should you start listening first?

Starting auscultation at the apex of the heart is essential because it is where the sounds of the heart valves are often best heard, particularly the mitral valve. The apex is located at the point of maximal impulse (PMI), typically found in the fifth intercostal space at the midclavicular line. By beginning at this location, the practitioner can easily assess the heart sounds that reflect the function of the left ventricle and the left atrium, which are critical in diagnosing conditions like heart failure.

Additionally, proceeding from the apex allows for a logical, organized approach to the rest of the heart sounds, as it serves as a central reference point. Once auscultation at the apex is complete, the examiner can then move to the base of the heart and other auscultatory areas to gather a comprehensive understanding of the heart's condition.
