What should you do to better feel the PMI during examination?

Laying the patient on their left side is the most effective position to better palpate the point of maximal impulse (PMI) during a cardiac examination. This position brings the heart closer to the chest wall, which enhances the ability to feel the PMI, typically located at the fifth intercostal space in the midclavicular line. By having the patient assume this lateral position, the heart's apex shifts slightly, making it more accessible for palpation.

Other positions or methods may not provide the same clarity or may obscure the PMI. For instance, while having a patient sit up can be beneficial for assessing heart sounds, it does not significantly enhance the palpation of the PMI. Palpating through clothing can also dampen the tactile sensation needed to feel the PMI accurately. Using a stethoscope with high pressure is not related to palpation and does not facilitate the feeling of the PMI, as it will not change the positioning of the heart in relation to the thoracic wall.
